Angiographic studies of the arteries adjoining the knee in 25 patients show extensive kinking and stenoses of the popliteal artery and less frequently of the distal femoral artery during flexion of the knee joint. This is due to the loss of elasticity with increasing age forcing the vessel into a tortuous course during shortening of the pathway of the popliteal artery with knee flexion. Independent of the principle of the different stents available they probably will not increase the contractility of the stented vessel in the longitudinal axis. Background We aimed to compare the performance of the recent CASTLE score to J-CTO, CL and PROGRESS CTO scores in a comprehensive database of percutaneous coronary intervention of chronic total occlusion procedures. Methods Scores were calculated using raw data from 1,342 chronic total occlusion procedures included in REBECO Registry that includes learning and expert operators. Calibration, discrimination and reclassification were evaluated and compared. Results Mean score values were: CASTLE 1.60±1.10, J-CTO 2.15±1.24, PROGRESS 1.68±0.94 and CL 2.52±1.52 points. The management of patients under the effect of antiplatelet or anticoagulant agents is a common challenge in thoracic surgery. Their temporary interruption or continuation needs a balanced assessment between the risk of thrombosis (interruption) and bleeding (continuation). Moreover, most patients must receive an anticoagulant for thromboprophylaxis (mainly a low-molecular-weight heparin). So, it is important to have in mind all surgical and anaesthetic implications of these drugs to take the optimal decision in each case.
